What is the ACT?
This survey was designed to help you describe your asthma—
how it makes you feel and what you are able to do. It will also help you
determine if your asthma is really controlled.

The purpose of this statement is to inform you about how the Asthma and Allergy Foundation of America (AAFA) intends to use information submitted by you about yourself or your child, if submitted on behalf of your child, as you utilize ACT (Personal Assessment and Control Tool). ACT is provided by AAFA for educational purposes as a public service.
AAFA is not a health care provider and any information received through ACT is not intended to substitute for professional medical advice. ACT generates reports based on information that you enter into the system. The more information you enter, the more you will learn from the ACT reports. AAFA recommends that you share and discuss the reports with your health care provider.
Registering for ACT indicates your understanding of AAFA's Privacy Statement for ACT and acceptance of its policies on use of the data you submit for yourself or your child. Please review the following statement before registering for ACT.
QualityMetrics will aggregate, that is, combine the "health information" you provide with that of other ACT participants, without linking it to any participants' personal contact information provided during registration. QualityMetrics will provide AAFA with monthly statistical reports on the number of new users, repeat users, total users and anonymous, aggregated reports of all participants' "health information". The aggregated data that AAFA receives will not be linked to any personal contact information, thus cannot linked to you personally.
After completing the ACT, you will have an opportunity to provide additional information about you through "pop up windows" or links to AAFA's website. For example, you may be asked to provide your email address directly to AAFA and your permission for us to send an email message to you. This would be for the sole purpose of reminding you to retake the ACT assessment tool three to six months later and receive a follow-up report. The email address you provide directly to AAFA will not be linked to any health information you provided through the ACT program. You will also have an opportunity to provide additional information in order to receive specified services and information from AAFA. If you choose to provide this information directly to AAFA, it will be used only for the purposes for which you have agreed.
